Requirements 

Initial

  1. Duly accomplished Application Form
  2. Letter of Intent
  3. Research Proposal
  4. Company/Institution/Organization/Agency Profile
  5. Application/Processing Fee
  6. Proof of Financial Capability in the form of credit lines or cattle ownership certified by the concerned Municipal Treasurer

Supplemental

  1. Prior Informed Consent (PIC) /Free and Prior Informed Consent (FPIC) Certificate from resource providers following the procedure for obtaining the PIC/FPIC
  2. Documentary Proof of Compliance with other relevant requirements under Joint DAO-DA-PCSD-NCIP 2005-01
  3. Summary of agreed terms of benefit sharing
  4. Letter of acceptance from local collaborator
  5. Strategic Environmental Plan (SEP) clearance from Palawan Council Sustainable Development (PCSD), if bioprospecting is to be conducted within the Province of Palawan
  6. Other requirements that may be required by concerned Government Agencies

Application for Permit for Other Uses of Wildlife

 

Requirements 

Exhibit

  1. (2) Duly accomplished application form, both with recent 2” x 2” photo of the applicant
  2. In case of animal shows using wildlife, Clearance/Registration from the Animal Welfare Division of the Bureau of Animal Industry pursuant to Republic Act 8485 Animal Welfare Act
  3. In case of imported animals, no importation shall be permitted except upon deposit of a bond with PAWB in an amount equal to the transport cost of the animals back to the Owner-country of Origin. Import/Re-export permits shall be secured in accordance to DAO 2004-55 Section 8

Education

  1. (2) Duly accomplished application form, both with recent 2” x 2” photo of the applicant
  2. Letter request specifying the species, quantity and areas of collection or area, subject of documentation, as the case may be
  3. Prior clearance from the affected communities, i.e. concerned LGU’s, recognized Head of Indigenous Cultural Communities (ICC) in accordance with RA 8371, or Protected Area Management Board (PAMB)
  4. Citizenship verification papers, if citizenship is by Naturalization

  5. Payment Fee in accordance with DAO 2004-55 Section 9

  3. Citizenship verification papers, if citizenship is by Naturalization
  4. Prior clearance from the affected communities, i.e. concerned LGU’s, recognized Head of Indigenous Cultural Communities (ICC) in accordance with RA 8371, or Protected Area Management Board (PAMB)
  5. Names and Addresses of Authorized Collectors/Trappers
