1. Duly accomplished Application Form, together with the proper signature
2. Certificate of Registration of the Peoples Organization
3. List of Officers or in the case of Indigenous People, List of Council of Elders or other similar governing bodies
4. Certified List of Members, including the Address and Complete Name of Spouse
5. Resolution from the members of the Peoples Organization allowing its President or Head to file the PACBRMA application
